The CDDP was created during the 2006 Legislative Session with the passage of House Bill 371, which was sponsored by Representative Gayle Harrell.  The purpose of the CDDP is to provide access to the drugs and supplies used to treat cancer to patients who are uninsured and do not qualify for Medicare, third-party insurance or any other state or federal programs.  Section 499.029, Florida Statutes, authorizes the donation of cancer drugs and supplies by any person or entity to a participant facility for re-dispensing to an eligible recipient.

Forms – Links to the forms required to participate in the CDDP.  All forms must be maintained by the participant facility for a minimum of three years, and must be available for inspection at all time.

  • Notice of Participation and Withdrawal (pdf - 25kb) – This form must be completed by all Class II Pharmacies that wish to become a “participant facility” in the CDDP.
  • Donation and Destruction Record (pdf - 25kb) – This form must be completed by the donor when cancer drugs and supplies are donated to a participant facility.  The form must also be completed if and when donated drugs or supplies are destroyed.
  • Recipient Record (pdf - 19kb) – This form must be completed by an person who receives drugs or supplies for the CDDP.

Participant Registry (doc - 34kb) –Listing of all Class II Institutional Pharmacies that are participating in the CDDP, and are accepting, storing and re-dispensing donated cancer drugs and supplies.
