Requirements:
For requirements to reactivate your license, inactive to active, contact
the Board Staff. If you are adding or
removing a status, such as paying a delinquency fee or changing a
license status please see the fee schedules included with your renewal
notice or contact Medical Quality Assurance Call Center at (850)
488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET.
You may renew
your license utilizing FLHealthSource.com on the Internet or by mail using the U. S.
Postal Service.
E-Renewal:
Renewal cycle closed.
Address Changes may be made online at any time.
In order to use e-Renewal, your license must be in a clear/active or clear/inactive status (delinquent licenses and fees cannot be processed on line) and use a credit card. You may renew your license utilizing FLHealthSource.com during the renewal period via the Internet or by mail using the U. S.
Postal Service.
E-Renewal allows you to make address changes. E-Renewal does not allow you to renew online by adding or removing a status, such as paying a delinquency fee or changing a license status. E-Renewal will require the following information:
A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
