Blank Renewal Forms: : To receive a replacement renewal notice please contact the Medical Quality Assurance Call Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET.

In-Service Hours:  CNAs are required to obtain in-service hours (a total of 24 in-service hours in calendar years 2006 and 2007) and maintain records of attendance of in-service hours for a period of 4 years.
You may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal Service

MQA Online Services: Renewal cycle is now open. To renew your license online, go to FLHealthSource.com . Address Changes may be made online at any time.

To renew online your license must be in a clear/active or clear/inactive status. Delinquent licensees cannot renew online.  The only form of payment accepted is a credit card.   You may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal Service.

MQA Online Services allows you to make address changes. However, does not allow you to renew online by adding or removing a status, such as paying a delinquency fee or changing a license status. MQA Online Services will require the following information:

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
