Renewal for Opticianry

Current licenses will expire at midnight, Eastern Time, on December 31, 2008.

The process of renewing a license may take two (2) to three (3) weeks.  Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.

Delinquent licenses expiring on December 31, 2006 become null and void on January 01, 2009.

* If you are requesting Delinquent Inactive to Active, you will be required to submit documentation verifying 20 hours of continuing education for each biennium your license was on inactive status. Reactivation Requirements:  For requirements to reactivate your license, inactive to active, contact the Board Staff. If you are adding or removing a status, such as paying a delinquency fee or changing a license status please see the fee schedules included with your renewal notice or contact Medical Quality Assurance Call Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET.

You may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal Service.

E-Renewal: Renewal cycle closed.  Address changes may be made at any time.

In order to use E-Renewal, your license must be in a clear/active or clear/inactive status (delinquent licenses and fees cannot be processed on line) and use a credit card. You may renew your license utilizing FLHealthSource.com during the renewal period via the Internet or by mail using the U. S. Postal Service.

E-Renewal allows you to make address changes. E-Renewal does not allow you to renew online by adding or removing a status, such as paying a delinquency fee or changing a license status. E-Renewal will require the following information:

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.

IMPORTANT RENEWAL INFORMATION

BOARD'S POSITION: Until you receive your printed license, it is the Board's position that you keep practicing as long as you have documentation that clearly shows a timely renewal was submitted. However, your employer may require you to have possession of your license to continue practicing. To ensure you receive your license prior to the expiration date, you should allow four to six weeks for processing, printing, and mailing.

MEDICAL ERRORS REDUCTION: Additional requirement of continuing education for initial licensure and renewal of license: 2 hours in the area of medical error prevention is required for initial license and renewal of license. This requirement is not in addition to the continuing education hours required for renewal, but a part of.
