Renewal for Massage Establishment (MM)

Current licenses will expire at midnight, Eastern Time, on August 31, 2007.

The process of renewing a license may take two (2) to three (3) weeks. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.

Reactivation Requirements: For requirements to reactivate your license, inactive to active, contact the Board Staff.

E-Renewal:  Open for renewal cycle from 4/26/07 through 8/31/07, midnight. Address changes may be made online at any time.

In order to use e-Renewal, your license must be in a clear/active or clear/inactive status (delinquent licenses and fees cannot be processed on line) and use a credit card. You may renew your license utilizing FLHealthSource.com during the renewal period via the Internet or by mail using the U. S. Postal Service

E-Renewal allows you to make address changes. E-Renewal does not allow you to renew online by adding or removing a status, such as paying a delinquency fee or changing a license status. E-Renewal will require the following information:

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
