General Renewal Information and Other Forms

NOTE: Florida Statute 766.314(6)(b)2 states that any physician who has not satisfied the annual Neurological Injury Compensation Association (NICA) assessment fee shall not renew any license to practice until such time as the judgment is satisfied in full. To ensure a smooth renewal process, be sure to take care of your judgments prior to your renewal.

Great news! Effective with the 2008 renewal, the renewal fee has been reduced to $360 (plus the unlicensed activity fee and the background check fee). Be on the lookout for the renewal notices, which will be sent on October 4, 2007 for those licenses expiring on January 31, 2008.
