Renewal for Medical
Doctors Public Health
Current licenses will expire at midnight, Eastern Time, on January 31, 2008.
The
process of renewing a license may take two (2) to three (3) weeks. Initiating contact to confirm the receipt of fees or the status of
your license prior to this time will not expedite the renewal process.

If dispensing add $100.00 to total.
Practitioners who fail to renew on or before 1/31/08 will become null and void on 2/1/08.

delinquency fee or changing a license status, please see the fee schedules
included with your renewal notice or contact Medical Quality Assurance

a.m. to 5:00 p.m. ET.
Blank Renewal Forms
You
may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal
Service. FLHealthSource.com is
available to licensees that
have no changes.
FLHealthSource.com Address changes may be made online at any time.
