Renewal for Diagnostic Radiological Physicist

Current licenses will expire at midnight, Eastern Time, on January 31, 2009.

The process of renewing a license may take two (2) to three (3) weeks. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.

Delinquent licenses that expired on January 31, 2007 become null and void on February 1, 2009.

Reactivation Requirements: For requirements to reactivate your license, inactive to active, contact the Board Staff. If you are adding or removing a status, please see the fee schedules included with your renewal notice or contact Medical Quality Assurance Call Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET.

E-Renewal: Renewal cycle is open. Address changes may be made online at any time.

In order to use e-Renewal you must use a credit card. You may renew your license utilizing FLHealthSource.com during the renewal period via the Internet or by mail using the U. S. Postal Service

E-Renewal will require the following information:

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
