Renewal for Midwifery

Current licenses will expire at midnight, Eastern Time, on December 31, 2009.

The process of renewing a license may take two (2) to three (3) weeks. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.

Reactivation Requirements: For requirements to reactivate your license, inactive to active, contact the Board Staff.

Delinquent licenses expiring on December 31, 2007 will become null and void on January 1, 2010.

* If you are adding or removing a status, such as paying a delinquency fee or changing a license status please see the fee schedules included with your renewal notice or contact Medical Quality Assurance Call Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET.

You may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal Service.  In addition to mailing your renewal notice and renewal fees, you must attach the Emergency Care Plan (pdf - 46kb) and the Financial responsibility form (pdf - 15kb) to be renewed.

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
