Current licenses will expire at midnight, Eastern Time, on September 30, 2008.
The process of renewing a license may
take two (2) to three (3) weeks. Initiating
contact to confirm the receipt of fees or the status of your license
prior to this time will not expedite the renewal process.

on 9/30/06 become null and void on 10/1/08.
Reactivation Requirements: For requirements to reactivate your license, inactive to active, contact the Board Staff. If you are adding or removing a status, such as paying a delinquency fee or changing a license status please see the fee schedules included with your renewal notice or contact Medical Quality Assurance Call Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to 5:00 p.m. ET
You may renew
your license utilizing FLHealthSource.com on the Internet or by mail using the U. S.
Postal Service.
FLHealthSource.comallows you to make address changes, update your profile and/or renew your license. However, it does not allow you to renew online if you are changing your license status. To renew your license, update your profile and/or change your address, you will need your user ID and Password. If you do not know your User ID and/or Password, you can also login by answering a set of security questions.
A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
