Renewals for Osteopathic Physician (OS)

Current licenses expire at midnight, Eastern Time, on March 31, 2012.

The process of renewing a license may take two (2) to three (3) weeks. Initiating contact to confirm the receipt of fees or the status of your license prior to this time will not expedite the renewal process.

Reactivation Requirements: For requirements to reactivate your license, inactive to active, contact the Board Staff.

If you currently dispense, please add an additional $100.00 to the active renewal fees indicated above.

Delinquent licenses expiring on 3/31/2008 became null and void on 4/1/2010.

You may renew your license utilizing FLHealthSource.com on the Internet or by mail using the U. S. Postal Service. In order to use e-Renewal, your license must be in a clear/active or clear/inactive status (delinquent licenses and fees cannot be processed on line) and use a credit card.

A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.
