renewal notice and a check or money order made payable to the Department
of Health to:

Renewal notices contain special
coding in several areas that are unique to each licensee.
Licensees who do not receive their renewal notice should NOT copy
another licensee’s renewal notice. If a licensee does not receive a
renewal notice within 75 days prior to the expiration date of their
current license, they should call the Medical Quality Assurance Call
Center at (850) 488-0595 press 3, Monday through Friday from 8:00 a.m. to
5:00 p.m. ET.
