Attention Medical Doctors and Doctors of Osteopathic Medicine: VeriDoc is a computer system that contains physician license and license discipline information that is provided by boards of medicine in the United States and its territories. Florida is a participating member of VeriDoc.
Licensure verifications to other state medical boards can be sent through VeriDoc electronically in a minimal amount of time. Attention Nurses: NURSYS™ (Nurse System) is a computer system
that contains nurse license and license discipline information that is
provided by boards of nursing in the United States and its territories.
NURSYS™ receives regular updates of nurses’ personal (name, address,
etc.) and license information from participating boards of nursing.
Florida is a participating member of NURSYS™. You may fill out the form
online at Nursys License Verification Request or register toll free by phone at
(866) 293-9600.
License Certification: Specific document certifying licensure status and disciplinary history, prepared by a representative of the Division of Medical Quality Assurance and bearing a seal. This document is generally required for applicants seeking licensure in other states and for use in court proceedings. There is a $25 fee for this service in accordance with Rule 64B-4.001.
Certification of Non-Licensure: Certification may also be
provided stating an individual is not a licensee. This is written
certification that there is no record of licensure for an
individual/facility, under the name and/or spelling of the name
provided, in the profession requested.
