transcripts, examination questions, answers, papers, grades
and grading keys, which are confidential and exempt from s.
119.07(1) will be withheld pursuant to Chapter 456.057,
Florida Statutes. Social Security numbers will also be
redacted pursuant to 42 U.S.C. 405(c)(2)(C)(vii)(1).
How to obtain copies of documents: Copies of Licensure, Disciplinary Orders,
and Declaratory Statements may be
obtained by completing one of our online forms. When
using the online forms, be sure to indicate whether you require the documents be certified
as official records.
Certification is a statement, under seal, that the
documents are true and correct copies of records on file
with the Department. If certification is requested on the
order form, a $25 fee will be charged, in addition to the duplicating
charges. Certification of the requested records will
not be done unless specifically requested.
If charges are incurred, an invoice will be mailed with the material and
payment will be expected within thirty
days.
- Application Documents - The
application file includes the application for licensure,
supporting documents submitted at the time of initial
licensure, and any subsequent updates to the file, excluding
those documents deemed confidential under Florida
Statutes. These documents are maintained, in the normal
course of business, as a part of the application file for any licensee
of the Division of Medical Quality Assurance.

- Declaratory Statements - A declaratory statement is a
means for resolving a controversy or answering questions
or doubts concerning the applicability of statutory
provisions, rules, or orders over which the board, or
department when there is no board, has authority, pursuant
to rule 28-105.001, Florida Administrative Code. A
petition for declaratory statement may be used only to
resolve questions or doubts as to how the statutes, rule,
or order may apply to the petitioner's particular
circumstances.

- Indicate the type of documents (Discipline or
Licensure/Application) you are requesting .
- For Discipline or Licensure/Application requests, Include the full name and license number of the
individual(s) to be researched.
- For Declaratory Statement requests, include the
subject, petitioner, final order number and the date the
order was filed for each declaratory statement you are
requesting.
