On April 18, 2006, in accordance with Rule 64B8-51.007(3), F.A.C., the exam fee for the electrolysis exam has been changed from $300 to $150. An electrolysis exam applicant whose application is received in our office on, or after, April 18, 2006, will only be required to pay the new $150 exam fee.

Licensure by Examination

An applicant must submit the application, fees and following documentation:

  • Fees: $355
    • ($100 application processing fee + $150 examination fee + $100 licensure fee + $5 unlicensed activity fee)
    • All fees are payable by check or money order made out to the Department of Health. DO NOT SEND CASH.
    • The application fees are non-refundable. If requested, the licensure fee may be refunded to you if you are denied licensure or if you decide to withdraw your application.
    • If incomplete, the application and fee may not be used for more than one year from the date of original submission of the application and fee.
       
  • Proof of Date of Birth – may use either:
    • Copy of driver’s license
    • Birth certificate
    • Current passport
       
  • Photo
    • One 2x2 photo (within the past six (6) months) No Polaroid photos.

  • Proof of High School Education
    • A copy of the actual diploma, GED, or high school transcript that shows the date of graduation is acceptable.
    • If in a language other than English, a translation must be submitted.

  • Proof of Electrolysis Training
    • An official transcript from an approved electrolysis training program. If not approved, a curriculum outline and a letter from the director of the program are required.
      • 120 hours minimum academic training
      • 200 hours minimum practical experience
    • The Electrolysis Council approves no home study or correspondence school. For more information contact MQA_Electrolysis@doh.state.fl.us by email or by telephone at 850.245-4373 ext. 2510.
  • Additional Education Requirements
    • Prevention of Medical Errors: A two (2) hour Prevention of Medical Errors education course is required for initial licensure.

  • Verification of License in Another State
    • This form must be submitted for each electrologist and health related license or certificate currently or ever held. All verifications must be sent to the council office directly from each respective state.
