Public Health Certificate authorizes the holder to practice only in conjunction with employment duties within the Department of Health and automatically expires when the holder’s relationship with the department is terminated.

Applicants applying for a Public Health Certificate must:

  • Be a graduate of an accredited medical school and hold a master of public health degree or be board eligible or certified in public health or preventive medicine.

  • Submit a letter of intention of employment from the Secretary of the Department of Health

  • Submit Specialty Board Certificate in Public Health or Preventative Medicine

  • Meet the requirements in section 458.311(1)(a)-(g) and (5) Florida Statutes
