21 years of age when applying for licensure

Submit application to the Council office

Proof of Liability Insurance in the amount of
$100,000 per claim and $300,000 minimum annual aggregate.

Must have completed one of the approved midwifery
programs.

Submits a valid certificate or diploma from a
foreign institution of medicine or midwifery or from a midwifery
program offered in another state which bears the seal of the
institution or is otherwise authenticated, provided the requirements
are deemed substantially equivalent to or exceeds those established
by Chapter 467 and Rule 64B24, Florida Administrative Code

Payment of all applicable fees

Must take and pass the State examination (unless
North American Registry Examination for midwives was taken and
passed prior to making application for state licensure)

A foreign trained applicant must provide a certified
translation of the certificate or diploma earned from the foreign
institution of medicine or midwifery

The document which renders the foreign trained
applicant eligible to practice medicine or midwifery in the country
in which that document was issued

Clarification if there is any deviation as to how
the applicant's name appears on the face of documents in support of
the application
-
Evidence of a passing score on the licensure
examination

Those applying from another state must provide a
valid unrestricted certificate or license to practice midwifery must
successfully complete an approved midwifery program.

Submit an authenticated copy of the certificate or
diploma awarded by the midwifery program which was approved by the
certifying body of the state in which it was located

A copy of the other state's laws and rules under
which the applicant's certificate or license was issued

Official transcripts from the midwifery program
which document classroom instruction and clinical training
-
Passed the college level academic scholastic test (CLAST)
or taken three college-level credits each of math and English, or
can demonstrate competencies in communication and computation by
passing the College Level Equivalent Proficiency (CLEP) test in
communication and computation.
