Florida Council of Licensed Midwifery
One-Time Fee Assessment Notice

On October 12, 2007, the Council voted to impose a one-time assessment of $250.00 that must be paid to the Department of Health on or before December 31, 2008 by each active, inactive and delinquent status licensee. Failure to comply with this assessment shall constitute a citation violation as set forth in Section 456.077, Florida Statutes.

To use the online system, please log onto www.flhealthsource.com and follow these steps:
  1. Select Licensee/Provider
  2. Select Practitioner Login and log in
  3. Click One Time Assessment

The system will be available for payment until midnight, Eastern Standard Time (EST), December 31, 2008.

Public School Volunteer Practitioner Program - The Public School Volunteer Health Care Practitioner Act (s. 381.00593, F.S.) was enacted during the 2002 legislative session. The goal of this program is to make a higher quality of health care available in Florida's public schools by encouraging health care practitioners to volunteer their services. This program is implemented by the Department of Health and the Department of Education, in coordination with the "School Health Services Act", s. 381.0056, F.S.
