Occupational Therapist/Assistant by Examination or Endorsement
The $180 fee must accompany the application or the application will not be processed.
The licensure fee ($75) may be refunded to you if you are denied licensure or if you decide to withdraw your application.
Please make your personal check or money order payable to the Department of Health and send the fee, application and appropriate documentation to:
