Reporting Requirement - Section 456.063(3), Florida
Statutes (pdf), states: Licensed
health care practitioners shall report allegations of sexual misconduct to the department, regardless
of the practice setting in which the alleged sexual misconduct
occurred.

include your name, license number, social security number, and
your old and new address.

Insurance Portability and Accountability Act (HIPAA) Information
Professional Assistance - The Professional Resource Network (PRN)
is available to provide assistance to those practitioners
regulated by the Department of Health. The Program’s purpose is to
ensure the public health and safety by assisting practitioners who
may suffer from one or more of the following: Chemical Dependency;
Psychiatric Illness; Psychosexual Illness - including Boundary
Violations; Neurological/Cognitive Impairment; Physical Illness;
HIV Infection/AIDS; and/or Behavioral Disorders. If you have
reason to believe that someone regulated by one of the Boards
under the Department of Health is impaired, be a friend, call PRN
(your anonymity will be protected), or have them call PRN, to
obtain help. Ignoring the problem supports their disease and not
