Accountability Act (HIPAA) Information
Public School Volunteer Practitioner Program - The Public
School Volunteer Health Care Practitioner Act (s. 381.00593, F.S.) was
enacted during the 2002 legislative session. The goal of this program is
to make a higher quality of health care available in Florida's public
schools by encouraging health care practitioners to volunteer their
services. This program is implemented by the Department of Health and
the Department of Education, in coordination with the "School Health
Services Act", s. 381.0056, F.S.

Obtain a Written
LicenseCertification
Duplicate License - For a duplicate license, please send
your current license, or a notarized affidavit stating your full
name, your license number, including the two digit alpha prefix
and why you do not have your current license, along with a $25.00
check or money order made payable to the Department of Health.
Please mail your request and fee to Department of Health, MQA,
Board of Pharmacy, 4052 Bald Cypress Way Bin #C04, Tallahassee, FL
32399-3254. You should receive a new license in approximately 6
weeks. |
