Podiatric Physician 

64B18-12.008 Registration Fee for Dispensing Practitioners

A podiatrist who dispenses medicinal drugs for human consumption for a fee or remuneration of any kind, whether direct or indirect, must register with the Board and pay a fee of $100.00 at the time of such registration and upon each renewal of licensure.

Breakdown of fees:
Application Fee - $100.00 (non-refundable)
Examination Fee - $200.00 (if examination is required add this fee)
*Fingerprint Card/Background Check $48.00 (non-refundable)
Unlicensed Activity Fee - $5.00
Initial Licensure Fee - $350.00

Total Fee without Examination - $503.00

Total Due - $703.00

*Effective October 1, 2007, the Division of Medical Quality Assurance will begin scanning fingerprint cards and electronically submitting fingerprints to the FDLE/FBI for background screening.  The FDLE/FBI fee is reduced to $48.00.
