not send cash. Return your
renewal notice and a check or money order made payable to the Department

To print your renewal notice online:
- Go to www.FLHealthsource.com
- Click the Licensee/Provider button
- From the left navigation menu, select Practitioner Logon.
- You will need your user ID and password to logon.
- If you do not know your user ID and password, you can log in by answering a set of security questions.
- Once you are properly logged in, you may print your renewal notice using the left navigation menu.
