Applicant Questions:
Q: The Board has certified eligibility for
certification/licensure. When can I initiate practice?
A: When you receive the letter with your license/certification number
authorizing practice.
Q: I am graduating soon. When should I apply for my
provisional license?
A: At least thirty (30) days prior to graduation you should submit your
application. Have your university send an official transcript reflecting
the masters degree to the Board office. If the transcript does not
reflect the masters degree, the SPA-2D Certification of Conferral of
Masters degree must accompany the transcript.
Q: After I have completed my nine (9) months of
professional experience, do I wait until I receive my CCCs from ASHA
before I apply for my active license.
A: NO! You need to immediately submit a completed SPA-1 application for
active licensure, along with the SPA-2B and SPA-2C evaluation forms
verifying completion of nine (9) months of CFY. If you have not
previously requested that an official report of your national
examination scores be sent to the Board office, you must request such
from ETS.
Q: What happens if my application for an active
license has not been approved prior to the expiration date of my
provisional license?
A: You must immediately cease to provide speech-language pathology or
audiology services and contact the Board office for further
instructions.

Licensee Questions:
Q: If I change my mailing address or employment site,
should I inform the Board office?
A: Yes. Each licensee is responsible for notifying the Board in writing
of the licensees current mailing address and place of practice.
Q: Do I submit verification of attendance/completion
of continuing education activities to the Board office?
A: No. Do not send CE information to the Board office. The licensee is
required to retain documentation showing completion of CE for two years
following the biennium for which they are applied. Audits will be
conducted at random to insure compliance with continuing education
requirements.
Q: Am I required to report certified
assistants/licensees to the Bureau of Investigations and Consumer
Services if I have personal knowledge that they are in violation of the
practice?
A: Yes.
Q: If I have a Florida teaching certificate, can I
provide private instruction as a speech-language pathologist?
A: No. You must have a license issued by the Department of Health, Board
of Speech Language Pathology and Audiology.
General Questions:
Q: How can I file a complaint against a licensee?
A: You may print the Complaint Form with the Authorization for Release of Patient
Records from the list below or call 1-888
419-3456 or (850) 414-1976 to request one by mail. To avoid delays in the process, please complete all sections of the form and attach the requested information. All complaints must be legibly written or typed, and signed.
To file a complaint on a non-medical related business or
profession, please visit the Department of Business and Professional Regulation.
Q: How can I check if someone is licensed?
A: You may use our License
Look-Up Search Screen. The best results often occur when the system receives the
least amount of information. Some techniques include only providing the county where the practitioner is located or the
last name or the first letter of his name, or select
a profession. If licensee is one of our profiled professions, there will be a
link at the upper right portion of the screen that says Practitioner Profile. This
will take you to practitioner reported information about their
education, specialty certification, etc.
Q: How can I get a list of licensee information?
A: To obtain an electronic listing of active and inactive licensee’s
mailing addresses and license status for DOH/ MQA regulated professions use
our Licensee Data Information service. Please note: The department does
not maintain email addresses, fax or telephone numbers in the normal
course of business.

F.S.,. will be withheld pursuant to Chapter 455.647(1), Florida Statutes. Social Security numbers will also be redacted pursuant to 42 U.S.C. 405(c)(2)(C)(vii)(1).
Q: What is unlicensed activity?
A: Providing speech-language pathology or audiology services in Florida
without an active, provisional license or assistant certification. Some
exemptions do apply see 468.1115 F.S.
Q: How can I become a Board member?
A: Board members are appointed by the governor and confirmed by the
Senate. You may apply by contacting the Governors Appointment Office.
Q: Who can attend Board meetings?
A: All Board meetings are open to the public and you are encouraged to
attend.
Q: How can I get a Board agenda?
A: You may request to be placed on the Board's Interested
Parties list through our Online
Application and Information Request . The Board routinely sends out Agenda Outlines to interested parties
7-10 days prior to an upcoming board meeting. Copies of the full "public" agenda
are also available, but are subject to a duplication fee. If you provide your
